Police Check Application Form

Due to our work with children, Plan conducts confidential national name checks for child-related offences on all staff and volunteers, and for sponsors who visit our programs overseas.

Instructions for Plan staff, volunteers and sponsors

  1. In Section A - put a cross next to "National Name Check"

  2. Complete Sections B and C. Plan will complete Sections D and E.

  3. Follow the instructions in Section F and G. Your completed form and original and photocopies of identification documents must be taken to an authorised person to sign as witness to your signature and certification of your photocopied identification documents. Authorised certifiers include Justice of the Peace, Australia Post or Bank employees (5+ years continuous service), Accountants, Teachers or Pharmacists. Ask us for a full list of certifiers.

  4. Attach a cheque or money order of $15.50 (a reduction is made for charities) payable to Victoria Police for the cost of the police check.

  5. Return to Plan, GPO Box 2818 Melbourne 3001

  6. Plan will forward your application to Victoria Police with payment.
