Police Check Application Form

Due to our work with children, Plan conducts confidential national name checks for child-related offences on all staff and volunteers, and sponsors who visit our programs overseas.

Instructions

1. Complete Section 1 only.
2. Have your signature witnessed at the bottom of the form (Section 4)
3. Attach a copy of your passport, licence or birth certificate.
4. Attach a cheque or money order of $13.70 (a reduction is made for charities) payable to Victoria Police for the cost of the police check.
5. Return to Plan, GPO Box 2818 Melbourne 3001
6. Plan will forward your application to Victoria Police with payment.
